Assisted clients with daily living needs to maintain self esteem,and increased general wellness.
Transported clients timely,and in coordinated manner to appointments,shopping venues,and entertainment events according to their determined schedules,and also based on their individual case management plans.
Helped clients manage and reach individual goals,supporting independent progression,and social skills.
Kept clients engaged in social networks,and communities for personal health,and growth
Respond to conflicts,utilizing crises intervention,and prevention when required.
Develop relationships with clients to provide support and housing stabilization.
Worked with other intensive case management workers to develop,and provide referrals for specialized support based on clients goals,needs,and choices.
Regularly visited clients, and updated assessment of support and stabilization needed,and collected waste to dispose.

Performed basic wound care by cleaning,and dressing wounds in cases of emergencies,or wound injuries for optimized hygiene.
Coordinated timely, appropriate animal transfers with shelters, rescue organizations and animal centers
