• In this role, I was a part of a pilot program, HISH (high intensity support at home). This program became a necessity within our community and was made into an integral community program. With the collaboration between 3 separate community based PSW programs, occupational therapists, nurses, and physiotherapists we were able to collaboratively streamline care based on individual needs and adapt care based on the progressive illnesses associated with the clients we cared for.

• In this role, I ensured that individuals are an active part of our community and participating in activities that are appropriate for their abilities. I actively encouraged my clients to progress in their Individual Support Plans and guide their next steps with compassion and patience. I documented with accuracy, their accomplishments and goals as well as any pertinent observations.
• I established a multi disciplinary environment to ensure that my clients had access to the best outcomes using new techniques that were available.
