• Assists with communicable disease outbreak investigations and disease control efforts, including gathering information, interviewing clients/patients and their contacts, and tracking all work in online databases and platforms
• Identifies cases, suspected cases, and contacts of communicable diseases. Interviews and advises clients/patients and their contacts who are at risk for, or have, a communicable disease; elicits the name of contacts and detailed information about the client/patient's personal behavior, including the location and nature of potential disease transmission
• Communicates guidance on testing for communicable diseases, home/self-isolation, and quarantine; calculates and communicate dates for start and end of home/self-isolation and quarantine period
• Employs motivational interviewing and other interviewing techniques to counsel clients/patients on behavior change/maintenance or lifestyle change actions that need to be initiated, educates about health risks, and promotes behavioral changes to reduce personal and community risks
• Motivates patients to adhere to treatment and prevention recommendations and identifies and implements interventions that support compliance and overcome barriers; refers to community resources and social services as needed

• Evaluated patients to identify and address wounds, behavioral concerns, and medically relevant symptoms
• Assisted patients with mobility needs, including moving to and from beds, organizing wheelchairs and preparing assistive devices
• Gathered dietary information, assisted with feeding, and monitored intake to help patients achieve nutritional objectives and support wellness goals
