• Assisting disabled clients to support independence and well-being
• Traveling to clients' homes to complete healthcare services and promote continuity of care
• Offering patients and families' emotional support and instruction in preparing healthy meals, independent living and adaptation to disability or illness
• Providing mobility assistance such as walking and regular exercising.

▪ Conduct intake interviews to determine eligibility for program enrollment and participation in the services
▪ Provide assessment of clients’ needs, advocacy, information and provide referrals to internal services as needed
▪ Provide appropriate external community referrals according to clients’ needs and assessment findings
▪ Support clients with translation and interpretation of documents ensuring accuracy of the converted information

 Provide assistance with completing form, letters, of invitation, application, translation and interpretation, as needed.
 Assess clients’ needs and determine the types of services required.
 Ensure that overall services targets set finder are met.

• Provide professional support to residents through constant companionship
• Provide assistance with activities or daily living such as feeding, lifts and transfers, bathing, oral hygiene meal planning and preparation in accordance with pre - established plan of care
• Continuously observe person(s) and their environments
• Report unsafe conditions as well as behavioral, physical and cognitive changes to a designated supervisor
• Complete and maintain related records and documentation such as communication books, progress report and client care notes perform light housekeeping duties.
• Assist with safe lift and transfers as identified in the resident care plan and respecting Hospice policy on lifts
• Routine turning and positioning as required
• Mechanical lifts must be used when the need has been identified in the client’s care plan – all mechanical lifts require the presence of two staff members

• Helped clients manage and reach individual goals, supporting independent progression and social skills.
• Transported clients to appointments, shopping venues and entertainment events according to determined schedule.
• Kept clients engaged in social networks and communities for personal health and growth.
• Assist with or dress the client according to his/her choice of clothing and timing
• Assist with bedtime care of the client
• Assist client to bathroom including ensuring safe and direct route when client and family may have rearranged the client room
