Provide harm reduction-based case management and money management services to formerly homeless adults with mental health conditions in a supportive housing setting. Support clients with maintaining housing and health.

  • Initiate and maintain contact with client's other providers as necessary.
  • Maintain and support a caseload of 100 clients with money management, representative payee services on site in a supportive housing residential building. Communicate and work with building property management staff to ensure clients' rents are paid accurately and timely. Fulfill all money management duties and Representative Payee policies.

  • Conduct Intakes for all new residents and set up services for money management in a timely and thorough manner.
  • Maintain contact with clients through outreach and continued support, to build rapport and trust.
  • Attend trainings provided by agency.
  • Assist in planning and facilitating special events for clients.
  • Attend monthly health and safety meetings to collaborate and provide staff and clients with information and training in the event of an emergency or disaster.
  • Facilitate weekly on-site groups for clients to encourage community building and decrease isolation.
  • Assist with weekly food bank and food voucher program.
  • Assess and implement appropriate steps during crisis intervention and physical health emergencies.

As required as part of the Community Health Worker Certificate Program, I worked part-time as an intern/volunteer at a local agency.

Assisted Homeless Youth Alliance staff in setting up and opening daily drop-in center for homeless youth. Worked with homeless youth and young adults aged 13-29 in the Haight Ashbury neighborhood of San Francisco. Provided a safe and welcoming space for homeless youth during day-time hours. Participated in street outreach in the Haight Ashbury district and Golden Gate Park, to offer hygiene supplies, snacks and harm reduction information to the youth residing there. Set up needle-exchange supplies for San Francisco Needle Exchange and volunteered when needed at the needle exchange.
