As a Health Care Navigator duties included but were not limited to the following:

  • Answering Incoming calls(50-60/day), from patients and caregivers, being the first point of contact from patient to Nurse and Providing excellent patient service to make sure needs were met with compassion/empathy and in a timely manner on both Canadian operations and for over 500 health centers and hospitals in the US
  • Creating patient charts by obtaining and confirming all key demographics and other health-related information and Reason for the call to enter it appropriately into the queue for Nurse/ Nurse practitioner call back.
  • Sending detailed notes in patient charts to respective health centers/Hospitals on behalf of patient noting
  • Taking calls on the Mental Health and addictions line and perform necessary actions in a timely and calm manor
  • Scheduling appointments / communicating with patients and their respective health centers
  • Precepting with New team members to show call process and to overlook as new members are taking their first calls
  • Doctor Paging
  • During the height of Covid-19 this role also included providing assistance to the province of Newfoundland by providing self-isolation information as well as completing a covid testing assessment to determine if testing was needed Based on symptoms ane/or close contacts.


After about 6 months, I was promoted to Shift Lead and these responsibilities included:

  • Being the go to for all agents and Nurses on shift and to be there to support, answer questions and to resolve any issues during shift.
  • Note all Callout's, shift pick up's and any changes on the daily break board and ensure staff have signed on for their respected shifts.
  • complete a shift report for each day, Which included any agent or chart errors, tech issues, meeting with agents and shift notes.
  • Monitoring the queue to ensure patient charts are added accurately and priorities are set based on symptoms and to fix Any issues that were present and follow up with agents if needed.
