I have worked with EHR, MEDI TEC, HPG and Connecting Ontario. I am familiar with OTN and the scheduling of appointments using it. Prior to SGS starting, I was a member of the Geriatric Outreach Team. I am familiar with community agencies and services that we often referred our clients to. I am familiar with registration and discharging of clients on the EHR. I have knowledge of the Mental Health Act, Health Care Consent and Substitute Decisions and PHIPA. I have worked effectively within the SGS team and have proven written and verbal communication skills. I have a high level of customer satisfaction in delivering my services in the past.
I am moving to the Lloydminster, Alberta area at the end of February 2025 to be with my daughter who lives there. I will have my vehicle and drivers license and would be able to travel to see clients in their own environment. I have worked in a hybrid situation for the past 6 years so I am very familiar with this.
