• Support clients in management of symptoms, i.e. development and utilization of personal coping strategies, community rehabilitation, and medication management.

▪ Assist clients in realizing personal daily living goals, both short and long term, to facilitate progression towards independent community living.

▪ Assist clients where appropriate with family communication and mediation.

▪ Assist clients as needed in developing and maintaining a positive working relationship with a General Practitioner, Dentist, Optometrist, Counsellor, Therapist, Psychiatrist, and other professionals in support of ongoing physical and mental wellness.

▪ Support clients in becoming active members of the community at large.

▪ Act as a positive role model for clients; lead by example.

• Daily documentation including logs, communication book, MAR sheets, etc
• Consistent communication with Supervisor

• Prepare month end Paperworks for the supported individual
• Ensuring all staff are following established protocols and providing feedback when necessary.

• Ensure that recreational and leisure activities are appropriate and of benefit to the individual and promote/participate in community outings
• Co-ordinate activities with the community resources to ensure that all clients participate as active members in community outings
• Provide accurate documentation of emergencies and accidents
• Help administer medication
• Monitored and maintained cleanliness, sanitation, and organization of assigned station and service areas.
