• Behavioral Health Management: Supported residents with psychiatric conditions, including mood disorders and anxiety, by applying evidence-based therapeutic techniques to improve their mental health and quality of life.
• Person-Centered Care: Created individualized care plans based on assessments and residents' preferences, fostering autonomy and respect for their dignity.
• Administered medications during medication pass, ensuring accuracy and timeliness.
• Managed fall risk for residents, implementing strategies to reduce injury risk.
• Monitored vital signs and documented them as part of regular care assessments.
• Provided wound care, assisting with patient recovery and comfort.
