• Assist in creating and implementing treatment and care plans for youth, ensuring monthly review and updates.
• Review youth activities and binders monthly, reporting discrepancies to the Executive team.
• Conduct monthly home assessments to maximize youth stability and health.
• Support annual performance reviews for Foster Parents and Alternates.
• Interact with Agency representatives, SW’s, justice system, schools, and community supports.
• Maintain a "High Risk" Youth Tracking list for vulnerable youth's safety and progression.
• Review and respond to incident reports and implement improvement opportunities.
• Ensure safe and clean work environments, attend monthly staff meetings, and enhance customer satisfaction through policy changes.
• Complete quarterly and annual CIC tasks as needed.
• Demonstrate exceptional caregiving skills and commitment to the well-being of foster youth at Level 5 proficiency.
• Provide a stable and nurturing environment that meets the specialized needs of high-risk and vulnerable youth.
• Implement advanced treatment and care plans with a focus on individualized support and positive outcomes.
• Collaborate effectively with agencies, social workers, and community supports to ensure the best care for foster children.
• Maintain accurate records, including MAR sheets, allowance records, and inventory, reflecting meticulous attention to detail.
