Patient Flow Nurse

• Responsible for screening new admissions, assessing patient charts, meeting with patients and families.
• Ensuring all necessary referrals are in place to maximize patients care throughout their admission with the goal of obtaining a safe discharge.
• Collaborated with the multidisciplinary team to develop individualized treatment plans for each patient.
• Work in liaison with HDGH intake nurses for RRP & CCC patients. Following up on new referrals, monitor patient progress, providing medical updates and facilitating transfers to the HDGH facility.
• Communicate with retirement homes, nursing home and group homes in regards to patient discharges.

Command Center Nurse

• Patient placement and maintaining the hospital flow.
• Assessing ED charts and findings appropriate beds on the appropriate servicing unit with also respecting infection control cohorts.
• Assigning beds and transferring patients based on acuity needs and infection control needs.
• Participates in daily meetings with all the hospital directors and managers.
• Provides reports of hospital census throughout the day for huddles and meetings
• Command center nurses are also the liaison between hospitals for transfers, repatriations and direct admissions.

• Able to confidently perform and educate other nursing staff on a variety of nursing skills such as trach care, chest tube care, iv starts, central line care, simple and complex wound care.
• Provided direct nursing care to a diverse patient population and critically ill patients in accordance with established policies, procedures, and standards of practice.
• Designated as "Responsible Person" ( charge nurse), this includes being a responsible team leader on the unit. RP include doing safety huddles, making assignments, replacing sick calls and communicating issues with the manager or after hours manager. Ensuring proper flow of the unit, facilitates staff with patient movements, discharges and admissions. Communicates frequently with management ei; staffing issues and any patient issues requiring escalation.

• Responsible and accountable for the provision of safe, effective, patient-focused nursing care to patients and families.
• Responsible for ongoing assessment and monitoring of the patient’s health status, identification of patient/family needs and the development of plans to meet those needs, the provision of direct patient care and the evaluation of care.
• Precepted 4th year nursing students.

• Promotes ambulation and physical activity in hospitalized patients
• Assist nurses in care of clients and work effectively with multidisciplinary team members
