• Anne is working as an HCA in a long-term care facility. Anne is working a night shift and has been assigned three clients: Mrs. Kim, Mr. Adams, and Mr. Lao. Two clients, Mr. Adams and Mr. Lao, share a room. During the shift, you are assisting Mrs. Kim with a position change when you hear a commotion in the next room. You walk into Mr. Adams and Mr. Lao’s room and notice that Mr. Adams has pulled out his catheter and there is blood on his pajamas. Mr. Lao has his nasal cannula off, and an open bottle of pills lying by his bedside table.
