Support Accreditation Canada readiness for medication management standard surveys in Fraser Health, Vancouver Coastal Health and Providence Health Care
Plan, develop, implement or evaluate regional medication safety initiatives such as nomenclature builds, order set reviews, clinical policy development, safe labeling and practice consults
Plan, develop, implement or evaluate quality assurance initiatives, policies and personnel competency assessment program for pharmacy compliance to sterile compounding standards (NAPRA, USP) and regulations

• Liaise with clinical operations leadership to develop and optimize clinical services in alignment with organizational strategic priorities and plans

• Perform financial planning, budgeting, evaluation and quality management activities to progressively deliver culturally safe and humble pharmacy services

• Plan, develop and implement clinical and distribution pharmacy professional practice initiatives to support nursing practice, education and safety in rural and remote BC First Nations health facilities
• Review and maintain medication formularies and relevant decision-making frameworks within multidisciplinary practice scopes, pharmacy and therapeutics committee frameworks, and clinical practice policies
• Plan, lead, develop, implement and sustain clinical pharmacy initiatives including infusion pump drug libraries, and novel COVID-19 therapeutics at rural and remote BC First Nations health facilities
• Perform gap analysis based on operational feedback, and responsively develop programs to support advance scopes of nursing practice
• Assess readiness, plan and develop pathways for nursing staff to access health information systems such as PharmaNet and Care Connect
• Plan, develop and implement medication safety and quality committee and FNHA's Hazardous Drugs Exposure Control Program in alignment with provincial working groups, FNHA Occupational Health and Safety and regional nursing operations
• Liaise across all organizational levels and nursing leadership to support development and optimization of pharmaceutical services and management strategy
• Plan and develop pharmacy support of mental health, substance use and harm reduction initiatives based on FNHA's framework for toxic drug crisis response
• Support FNHA's Registered Controlled Substances Officer (RCSO) to plan, develop, implement, and evaluate nursing professional policies, and clinical and operational practice for narcotic medication management
• Liaise with provincial health policy decision makers to identify and advocate for gaps and potential strategies to improve BC First Nations' access to equitable substance use care
• Represent FNHA and BC First Nations communities with the acknowledgement of the impacts of systemic racism, intergenerational trauma, and social determinants of health on Indigenous healthcare outcomes
• Advance BC First Nations' identified pharmacy priorities at federal and provincial pharmacy leadership tables for services and supports with principles of BC's Cultural Safety and Humility Practice Standard
• Advance nursing strategic priorities, and embed distribution and clinical pharmacy supports, opioid stewardship and quality assurance initiatives
• Develop strong strategic relationships with health system partners, facilitating communication and collaboration to advance improved health outcomes for Indigenous clients and communities in BC

• Coordinate a provincial brand conversion project in collaboration with PHSA and regional health authorities for tacrolimus, immunosuppressive agent used by 3400 solid organ transplant recipients in BC
• Leverage project management strategies to engage with key partners to assess implementation readiness, establish critical processes for client safety and clinician support during transition phases
• Develop and implement project materials including project charter, clinician and client information materials using standardized documentation tools
• Conduct comprehensive engagement and communications plan for transplant recipients, clinicians at BC transplant centres, post-transplant clinics, acute care hospitals and transplant partnered pharmacies across BC
• Inform and engage regional health authority teams including medication safety, pharmacy and therapeutics, production centres, pharmacy informatics to support readiness to meet project go-live dates and deadlines
• Plan, schedule and conduct clinician meetings and in-service rounds to communicate implementation methodology and project timelines
• Plan, schedule and conduct online and in-person client information sessions in collaboration with BC Transplant communications managers
• Coach and support of program clinicians throughout project's life cycle
• Collect and document key performance metrics to track status, progress and capture learnings through clinician and client feedback

• Provide distribution and clinical pharmacy services including consultation, medication management, and identification and resolution of drug therapy problems
• Perform clinical and dispensing assessments of prescriptions with the utilization of applicable pharmaceutical references and laboratory data for therapeutic suitability
• Provide consultation on drug interactions or drug therapy problems and liaise with prescribers and nursing for resolution and coordination
• Collaborate with prescribers, nurses and allied health care providers to facilitate timely and high-quality pharmacy services to clinic's clients
• Participate in client care rounds and respond to drug information requests from interdisciplinary team members to advance client care outcomes
• Participate in ongoing training, education, and quality improvement initiatives to contribute to progressive pharmacy services in the department
