Strathcona County Health Centre (Ambulatory Clinic/Home Care) and Fort Saskatchewan Health Centre (Home Care)
- Collaborate with interdisciplinary team members (ET Nurse, PT, OT, RT, Social Worker) to deliver comprehensive and optimal client care
- Conduct thorough assessments based on client needs, including head-to-toe, cardiac, and palliative assessments
- Provide ongoing wound care for clients with both simple and complex wounds, including the use of Wound Vac systems
- Administer Home Parenteral Therapy (HPT), including PICC line dressing changes, tubing, cap, and bag changes, as well as troubleshooting HPT pumps
- Administer IV medications via gravity and maintain IV sites for optimal care and patient safety
- Perform and educate clients on various clinical procedures, including catheter insertion/removal/irrigation, Clysis initiation and maintenance, PleurX drainage and dressing changes, ostomy and nephrostomy care, IVAD maintenance, blood draws, and more
- Act as case manager, coordinating with clients, families, pharmacists, and doctors to ensure comprehensive follow-up care, medication management, and consideration of family dynamics
