
- Strive to maintain contact with each client in order to deal with problems as they arise and remind clients of upcoming appointment / home visits.
- Organize and maintain complex records and reports manually and in computerized systems
- Promptly respond to all client communications from outside agencies
- Work effectively within a community setting and used appropriate resources and services
- Answer questions about services, benefits, and assist seniors in managing their health care
- Provides services and information via phone, correspondences, and home visits
