• Assist clients in developing a comprehensive individualized plan which reflects the client’s goals, values, skills, resources, and service requirements.
• Promote the health, independence, recovery, and community integration of persons with mental health and/or addictions challenges through advocacy, education, and the provision of a broad range of community-based and housing support services

- Conduct assessments, intakes and assist tenants in identifying any issues, problem areas, and relevant needs within their housing community and facilitate problem resolution to address issues.
- Facilitate, support, and encourage engagement of tenants in the ongoing management of their building and participation in tenant community.
- Work from an eviction prevention model to support tenants to maintain housing through regular follow up, case conference meetings, and liasing pertient supports to tenants.
- Develop Community Building programs to engage residents, encourage social interaction and inclusivity, and to support empowerment and self-sufficiency.
- Collect rent,establish payment schedules to manage any arrears, and follow up LTB Notices.

• Provide on-site supervision and direct service provision including Activities of Daily Living (ADLs) support with a focus on building a range of life skills, group facilitation, social recreational programming
• Ensure medication accuracy via medication monitoring, proper documentation, incident report for missed medication and medication check upon delivery
• Perform residential duties such as menu planning, food prep/cook, program planning ordering supplies, cleaning, coaching etc.
• Ensure the safety of all residents and staff members through conflict resolution, crisis prevention/intervention, risk management, assess mental stature, supportive counselling etc.

• Ensured that all client-residents are aware of and are following the C-19 prevention protocols during daily weekday visits.
• Ensure that all houses are fully supplied with PPEs and disinfectants
• Assess and monitor each client residents mental and physical changes for any decline in their overall health
• Collaborate with case-managers and other health care professionals in providing the best support for each client
• Supported clients with their day-to-day activities (ADLs) as well as medical and/on non-medical appointments

• Developed an IRP (Individual Rehabilitation Plan) for each individual client
• Implement behavioural and rehabilitation interventions to prevent decline in our client’s mental and physical health
• Supported clients with their day to day activities as well as medical and/or non-medical outings
• Collaborated with health care professional and other support workers to implement ongoing assessment
• Completed documentation in a timely and accurate manner
• Planned social gatherings for all clients to focus on the quality of life
