For my senior nursing capstone project, I focused on delirium prevention and management at the California Pacific Medical Center (CPMC) Acute Care for the Elderly (ACE) unit. Delirium is a common and serious condition among hospitalized older adults, often leading to longer stays, increased morbidity, and higher healthcare costs.
During my clinical rotation in the ACE unit, I had the opportunity to observe and participate in interdisciplinary care aimed at early identification and mitigation of delirium risk factors. My project involved assessing current nursing practices for delirium screening, evaluating patient and family education materials, and collaborating with the care team to implement evidence-based interventions such as frequent orientation, mobility encouragement, medication review, and environmental modifications to reduce sensory overload.
Through this experience, I gained a deeper understanding of the complexity of delirium in elderly patients and the critical role of nurses in recognizing subtle cognitive changes. It reinforced my commitment to holistic, patient-centered care and highlighted the importance of communication among healthcare providers and families to improve outcomes for vulnerable older adults.
