Provide individual and group counseling to clients with substance use disorders, using evidence - based practices like Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Contingency Management
Conduct thorough assessments to determine clients' needs, substance use history, and mental health status
Support clients in developing coping mechanisms and relapse prevention strategies
Work closely with medical professionals to monitor patients' physical health and medication-assisted treatment (MAT)
Maintain accurate documentation of patient progress, treatment plans, and case notes in compliance with HIPAA and other regulatory standards
Monitor Client Progress: Track client safety and well-being over time, adjusting safety plans and interventions as needed to ensure continuous improvement and stability.
Document and Report Case Details: Keep accurate records of client interactions, safety assessments, and incident reports, ensuring all documentation is up-to-date and compliant with organizational policies.
Conduct Follow-up Services: Ensure continued support through regular follow-ups with clients, reassessing safety needs, and making adjustments as necessary
Advocate for Clients: Act as a liaison between clients and service providers, advocating for client needs and ensuring they receive appropriate resources and care.
