Complete the admission, transfer and discharge of clients and pre-care conference
activities within assigned area(s) including Head-to-Toe assessment and systems
review. With the support and guidance of the Care Coordinator, liaise with health
authority representatives, pharmacy, paraprofessionals, physicians and other
stakeholders to ensure individual resident needs are addressed while including input
from residents and their families;
• Identify client risk factors including but not limited to falls, dysphagia, nutrition,
development of contractures, resident at risk for elopement or social isolation, and report findings for follow up appointments and assessment
Medications may be administered via PO, SC, IM, G/J Tube, topical ointments,
nebulizers/inhalers and suppositories. Medication administration includes observing,
monitoring, reporting and documenting therapeutic effects;
• Bring a Stethoscope and BP cuff with them to each client.
Perform nursing protocols and follow up on nursing / medical protocols including:
• Skin and wound care
• Braden Scale and Morse Fall Scale Assessments
• Bowel and bladder protocols initiated by a physician and assessed / documented by
nursing staff
• Catheterization (male/female)
• Specimen collection
• Pain management – assessing and monitoring medication effectiveness and
advocating for residents
• Infection control protocols
• Meeting safety standards, using equipment safely and accurate
