The Geriatric Program provides specialized geriatric mental health services to patients who experience persistent mental illness, dementia, or progressive cognitive impairment. The program focuses on maximizing independence and quality of life through the empowerment of patients with the goal of providing services aimed at rehabilitation and safe reintegration into a community of the patient's choice.

• Conducted independent risk assessment for self-directed violence, analyzing risk factors, protective factors, mental status and current triggers.
• Implemented evidence-based and best practice problem-solving interventions to establish protective measures for suicide prevention and self-harm.
• Developed collaborative safety plan with patient and family to prepare for discharge.
• Educated patients, families, and caregivers on diagnosis, treatment, and disease management, equipping them with tools to achieve and maintain optimal psychiatric and mental health.
• Documented all medications administered to mental health patients, following instructions from physicians and healthcare providers.
• Monitored patients prescribed psychotropic medications to assess medications' effectiveness and side effects.

• Monitored patient's respiration activity, blood pressure and blood glucose levels in response to medical administration.
• Communicated concerns regarding patients' status, care and environment to nursing supervisors, clinical care supervisors, and case managers.
• Answered patient and family questions to educate on optimal treatment procedures.
• Looked for physical, emotional, and symptomatic changes in patient condition and obtained necessary care for medical concerns.
