EXPERIENCE

-Arranged and conducted intake home visits with clients to assess eligibility for home care services

-Determine care needs for client to remain safely in their home and reduce care giver burden for families.

-Collaborated with hospital home care case coordinators regarding discharges from hospital to community 

 -Referrals to Community Therapy Services - occupational therapy or physiotherapy if concerns with clients safe mobility and transfers

-Referrals to Geriatric Outreach Service clinicians or Seven Oaks Day Hospital geriatrician for assessment, diagnoses and recommendations such as medication review for client’s who are exhibiting cognitive decline and or behavior concerns.

-Consulted, collaborated with the Long-Term Care Navigator to panel and prepare Personal Care Home applications

-Complaints were dealt with in a professional/calm and neutral way and concerns regarding HCA's was forwarded to their supervisor the Resource Coordinator, scheduling concerns were forwarded to the scheduling clerk.  

-Documentation in EHCR (electronic health care record) dated notes, regarding phone calls or other pertinent information

-Summarized home visit assessments in the Presenting Situation that encompassed a holistic view of client’s situation. 

-Ordered supplies/equipment for eligible client’s such as incontinent supplies, commodes, transfer belts, electric hospital beds, bed rails

 -Currently managing a caseload of 122 clients. 

-Covered for colleagues on vacation or on medical leave

-Conducted pre-admission social work assessments with hospitalized patients to determine potential fit for personal care home placements

- Administered Mini-Mental Status Examinations 

- Admission to PCH involved meeting with residents and their family members to review financial(notice of assessment from income tax) and legal documents, end of life care and Advanced Care Planning

-Conducted tours at Parkview Place PCH for interested/potential future resident and their family members, chaired annual case conferences with clients and professionals from multi-disciplines to discuss care plans and emerging issues,

 -Offered supportive counseling for clients
