Inpatient Physiotherapy (i.e Geriatric medicine unit, Orthopedic, General surgery, Vascular surgery, TCU, ED):

• Provide Physiotherapy services in acute care to patients with various conditions.
• Screen and assess patients, develop patients' treatment plan and goals, and implement plan according to conditions.
• Track patient progress and adjust approach as necessary.

Geriatric Day hospital (Outpatient)/ Geriatric Emergency management (Emergency Department):

• Developed strong relationships with referring physicians, ensuring seamless communication and coordination for mutual clients, needs and discharge planning.
• Assist patients in obtaining appropriate assistive devices under the Assistive Device Program.
• Participate in community outreach events to promote awareness of physiotherapy services and benefits for Geriatric population.
• Liaise with other healthcare professionals to exchange information about patients.
• Coordinate patient care with interdisciplinary team of medical professionals and specialists.
• Develop comprehensive treatment plans and goals to address individual patient needs in the community.
