Using the nursing process and working within the guidelines of Home Care Services, with a focus on
outcomes for independence, the Registered Nurse:
 Completes the initial assessment for Home Care clients requiring nursing service.
 Develops and maintains nursing care plans based on nursing assessment, available data and
analysis and evaluation/review.
 Performs clinical nursing skills in accordance with best practice guidelines and Home Care
and RQHR policies and procedures.
 Provides health teaching for clients and their supports regarding illness and appropriate
intervention; and independence where appropriate.
 Assists and involves clients and family members in planning client-centered care through
teaching, supporting and collaborating with other members of the interdisciplinary health care
team and community agencies; striving for client independence where appropriate.
 Encourages health promotion and illness prevention practices.
 Identifies when clients are in an emergency situation, provides interim emergency care and
assist clients to access emergency services when necessary.
 Arranges for required equipment and supplies.
 Maintains nursing documentation records according to accepted RQHR standards and policies.

• Cultivated a warm and compassionate atmosphere for patients receiving chemotherapy  in an infusion treatment facility.
• Completed chemotherapy administration as dictated by the physician's instructions on each patient's chart.
• Recorded all patient discussions and transactions in charts for future reference.
• Provided patient education prior to receiving new chemotherapy treatment
• Provide proper assessment on patients receiving chemotherapy
• Administer each chemotherpy as prescribed by physician in different routes, Intravenous, Subcutaneous, and Intravesical
• Ordered and read lab tests, x-rays, MRIs, Ultrasounds, and EKGs.
• Asked patients open-ended questions to garner information about side effects, mobility and energy levels, and then documented the information in their charts.

Primary nurse

• specialize in assessing, intervening and stabilizing a variety of trauma and illnesses with decisive action. Due to the range of medical conditions that may require urgent treatment and care,   as ER nurses I am knowledgeable about general as well as specific health issues.
• An emergency nurse must have the ability to make quick and accurate assessments about incoming patients, including both physical and mental health conditions.Time management is crucial in this job. As an emergency nurses, I must quickly ascertain the following information from incoming patients: Identify the medical problem, Document medical history, Check for any allergies and current medications, Obtain height, weight, body temperature, heart rate and blood pressure and Other responsibilities include moving patients, taking blood samples thru central lines, cleaning and bandaging wounds, administering medications and maintaining proper supplies of medical equipment. In addition, comply with protocols, procedures and safety policies of  Regina Qu'appelle Health Region. Also have done nursing preceptorship/mentor to 4 th year nursing student.

 Triage Nurse 

•  Trained in utilizing the CTAS (Canadian Triage and Acuity Score) and uses this information direct patient appropriately and in timely manner to be seen by ER Physician
• as a triage nurse I am the first point of contact for a patient entering the emergency room. Is responsible for gathering initial information from the patient such as their symptoms, age and current medical conditions. Also take vital signs, sometimes measure weight if necessary or check blood glucose level and ask for ECG or old charts from health records. 
• Also aware of the different protocols in place for if patient present with specific signs and symptoms which meet criteria to call such as MI Alert, Stroke Alert, Code blue etc.

 Charge nurse

•  perform such duties as delegating nursing assignments when needed, overseeing admissions and discharges, and Processing & monitoring each physician's orders and ensuring the flow of patient in the ER department, able to develop plan in patient flow and collaborate with management/unit coordinator/bedline/ward. able to utilize and coordinate with other health team and Ward staff/nurse, auxillary staff and porters and housekeeping in ensure safety and quality care to patient. 
• Direct other nurses on patient care, providing guidance and advice. Continue to motivate and lead nursing staff, able to work effectively with different personalities. Have the knowledge and experience to answer staff and patient questions on a wide range of topics.
• skilled in attention to detail (be on top of any critical results of patients with regards to their blood work and follow up with primary nurses as how patients development with treatments given), Good organizational and analytical skills (which involve in moving patient the department), Ability to quickly and accurately assess patients’ conditions, A caring and sympathetic manner, Emotional stability, Ability to efficiently handle emergencies and remain calm in stressful situations.

• Prepare and set up dialysis machine and properly prime the machine based on individual patient prescription prior to patient arrival.
• Review Physician orders, check medication and previous flow sheets, access section and check for allergies. Prepare to have report from ward regarding renal inpatient, and check inpatient chart orders
• Able to check correct dialyzer, correct bath and program settings
• Ensure thorough flow sheet assessment including vital signs, standing and sitting blood pressure, appropriate holding of heparin and pre-hemodialysis blood work based on assessment and the need for pre dialysis medication
• Donning Personal Protective Equipment properly prior to accessing hemodialysis access which includes Arteriovenous fistula and arteriovenous graft and central venous catheters.
• Initiates Treatment appropriately such as securing lines or connections and flipping kidney.
• Ensuring proper documentation such as entering vital signs, run date and observations, correct start up treatment time.
• Ensure have knowledge and capabilities in patient and Bay routines during hemodialysis Run of a patient. Reassessment and documentation every 30minutes as needed, work collaboratively with team member, correctly administer medication and appropriately administer blood products, saline flushes.
• Ensure able to troubleshoot dialysis machine alarms appropriately
• Able to identify and respond appropriately to change in patient status
• Process physician's order sheet, prepare for the next hemodialysis patient run and team leader checklist
• Ensuring safe discontinuation of patient hemodialysis treatment that includes proper PPE, drawing post blood work , appropriately removing needle from AVF/AVG and applies appropriate pressure, assessing for thrill while holding site, dressing of the site. Ensure appropriate lock and cap ports and labelling the ports
• Ensure proper documentation of patient vitals post treatment, post weight, giving report to ward and copy of run sheet of inpatient.
• Ensuring safe discharge of patient and document time of discharge and patient status upon discharge
• Participated in education days, including evidence-based practice project implementation, nursing competency development and nursing simulation activities as hemodialysis nurse

• As Lab faculty, responsible in instructing students to bring required documents during the Lab Assessment
• Reviewed with students the theory and technique required to complete the new assessment
• Reviewed Vital Signs, pain assessment, 60 second survey/assessment, basic lab value interpretation and application of skills during simulation
• Ensuring the students to have an opportunity to practice the new assessment/skills.
• Teaching students the abnormal signs and symptoms related to the disease process
• Ensure that students are applying critical thinking and appropriate nursing intervention during the simulation
• Facilitated Student in 2 groups and designate them into differrent role during Simulation such the Lead, Recorder, Assessor and Assist
• Discussing case studies related to disease process discuss during simulation and lab
• Monitor students lab attendance

Clinical Resource Nurse primarily involved in Educating, mentoring, and supporting Unit Staff. Promote patient Safety which includes but not limited to follow up with Occurrence report, educating, reviewing and demonstrating old and new equipment. Follow up with nurses regarding there recertification of Transfer of function (cardioversion, defibrillation and pacing), Intravenous Push medication every year, ER Protocols and standards to review every year. Promote educational opportunities to staff. Assist the educator with the June Education Days every year. Be of assistance to staff if there is any clarification with certain procedures and orders.
