Hennick Bridgepoint Hospital - Renal Unit (January - April 2024) - Semester 3 Clinical Placement. Under the supervision of my buddy nurse and clinical instructor, I observed and assisted in various patient care procedures, including performing assessments, collecting patient data and documentation, monitoring vital signs, administering medications, educating and supporting patients with multiple conditions, performing nursing procedures such as suctioning, catheterization, and tracheotomy care, and providing direct patient care and ADLs. In addition, I participated in interdisciplinary round meetings such as shift reports and huddles. I also attended in-service training on hemodialysis, maid, and mental health support.
Baycrest Hospital Geriatric Centre - Apotex unit (Sept-Dec 2023) - Semester two clinical placement. Under the supervision of my clinical instructor, my responsibilities are assessing and monitoring patients' health status and assisting with ADLs, monitoring vital signs, documenting patient information to gain hands-on experience and applying theoretical knowledge to real-world healthcare settings, administering medications under supervision, performing basic nursing procedures such as catheterization, wound care, assisting with transfers, mobility, and shadowing experienced healthcare professionals to learn new techniques and approaches to enhance my clinical skills and knowledge.
I provide care to clients with various needs, document their progress in treatment plans and educational activities, keep updated case records, maintain confidentiality, educate family members about resident care, and work collaboratively to ensure an inclusive atmosphere. I support clients' self-esteem and monitor their behaviour. I assisted with personal hygiene and ADLs. I assisted clients with disabilities with mobility and transfers, providing safety and well-being. I work closely with clinical staff to support therapeutic and behavioural plans for clients and maintain personal hygiene.
Assist in developing classroom activities. Observe students and reinforce positive behaviour. Supervise students during non-classroom time and arrange recreational activities. Collaborate with the lead teacher to identify and resolve students' issues. Assist students with learning disabilities. Observe and guide students' behaviour and implement safety protocols.
I assisted dementia clients with ADLs, assisted with support care plans and performed housekeeping tasks. I report client status to care managers and uphold solid bonds with clients, families, and care managers. I participated in recreational activities, kept accurate records, and helped with everyday living tasks. When necessary, I offered adaptive equipment and emotional support. I worked with interdisciplinary team members and created individual care plans based on assessment results and physician orders. I assisted with mobility and independence, monitored safety, and upheld daily living standards for clients. I collaborated with clinical personnel to support behavioural and therapeutic plans and monitored clients' health.
