- Schedule of all annual and quarterly RAI Assessments

- Manage, coordinate and complete the resident assessment tracking forms, Resident Assessment Protocols and care plans - - Provide LTCF RAI, Point Click Care, Point of Care and software training to all appropriate staff members

- Work in consultation with management and interdisciplinary team members to develop and utilize processes that ensure data integrity, efficient data collection, assessment completion and data utilization

- Assist with Resident Care Services/Intake Coordinator involving admission process, maintenance of facility census and maintenance of resident business files

- Facilitate annual resident Care Conferences and manage/coordinate other required assessments

- Active participation in the Continuous Quality Improvement/Risk Management, Long Term Care Committee

- Developed training materials for new hires, improving onboarding processes and staff retention.
