  • Utilize and apply extensive knowledge of case management principles, psychosocial rehabilitation, cognitive, behavioral, dialectical behavioural therapy, supportive, family and group therapy, discern which approaches are best suited to the situation and clinical impression;
  • Use substantive knowledge of numerous areas including, mental illness, cognitive functioning, medical co-morbidity, and substance abuse, analyze and interpret the information to formulate clinical impressions and develop treatment plans.
  • Respond and deal with emergency calls and walk-in clients; intake screening of new referrals for service; walk in clinic coverage as needed, determine intervention required while taking into consideration the degree of risk to client and/or others;
  • Gather information from interviews with the client, family and other professional involved (physicians, continuing care facilities, supported housing, community partners, caregivers, etc.) where appropriate;
  • Utilize independent professional judgment analyses of the presenting problem, set appropriate goals, select treatment options, provide relevant interventions, monitoring feedback, and adjust treatment, as required;
  • Examine the client's emotional situation, daily functioning, past and present life situations;
  • Work collaboratively with Physicians, other aligned Health Care Professionals, Seniors Resource Mental Health Team, Outreach, Child & Youth Psychiatry Unit, Student Wellbeing Teams, Primary Care, Out Landing Place, Mobile Mental Health and other services specif to supporting clients and their mental health.
  • Efficiently and accurately assess the person and situation to determine the potential level of risk, by completing Suicide Risk Assessment and Management with clients;
  • Work effectively with communities and service providers to create links for health promotion, prevention and service development to improve appropriate access;
  • Ensure assessment and treatment plan information is distributed according to departmental requirements and standards of client care;


Senior Social Worker (Foster Care/Adoption Assessment Practitioner)

  • Review SAFE assessment applications/questionnaires for foster care and adoptive applicants; Assign Desk Guide ratings on Psychosocial Inventory; Have initial meeting with the Provincial Adoption Coordinator to review an applicant(s)' information
  • Subsequent meetings with the Provincial Adoption Coordinator are also required to review/consult throughout the assessment process; Determine the applicant's potential to become a foster or adoptive parent and evaluate the type of child whose needs they can meet
  • Conduct risk assessments and home studies, as requested by the Director of Child Protection, on behalf of other provinces as per the Provincial/Territorial Protocol on Children, Youth and Families Moving Between Provinces and Territories
  • Develop an interview worksheet, prepare and complete home study interviews
  • Utilize structured framework for preparing and supporting Foster and Adoptive families
  • Facilitate group training sessions
  • Assist in the development of knowledge, skills and strengths, while understanding the importance of community supports in order to be successful as foster care or adoptive families
  • Respond to general inquiries from the public; keep statistics on Foster Care/Adoption Assessments and related services
  • Assist the Provincial Adoption Coordinator in the provision of adoption services to the public, Child and Family Services staff and the private authorized social workers and licensed liaisons
