Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan.

Assessment: through individual meetings on a regular basis and as part of Case Management team develop a detailed plan of action to meet the clients needs, set goals and find the necessary resources to meet those goals

Coordinate Care and Case Management of assigned case load as part of team, connecting participants to appropriate services

Conduct employability assessments of potential participants, this includes, but is not limited to assessing level of ability, readiness for work, physical and mental capabilities of participants, educational background, skills and identification of challenges and barriers to employment

Improved client outcomes by developing and implementing comprehensive case plans tailored to individual needs.

Write special incident reports detailing incidents that occur in Juvenile Hall.

Ensures care and safety for Probation youth during night hours.

Transports and prepares probation youth for transportation to medical facilities, courts and other locations.

Communicates and consults with probation officers, attorneys, families, law enforcement and social services agencies.

Explains, monitors and enforces facility rules and safety and security measures.

Documenting all progress and violations.

Meeting with offenders regularly to check for progress and ensure they're following the terms and conditions of Probation.

Coordinating support and services for probationers.

Prepared and maintained caseload for each assigned probation client.

Providing recommendations for each offender based on their situation.

Developing treatment and rehabilitation plans.

Using various techniques, including drug and alcohol tests, monitoring software and GPS monitoring to track probationers.
