· Clinical Role – To assist RPNs with assessment of clinically unstable residents – responsive behaviors, pain, falls, palliative, return from hospital, skin and wound, continence, new admissions etc.
· Attend Physicians’ rounds as necessary
· Provide coaching, mentoring and guidance to the Charge Nurses/RPNs
Oversee admissions
· As per schedule set by RAI-MDS Coordinator relieve the Charge Nurses/RPNs for unit duties to facilitate completion of RAI-MDS Assessments, documentation and care planning.
· Report high risk issues to DOC and ADOC (depending on the Floor assigned)
·Attend care conferences for high-risk residents or as required- to be directed by DOC
· Review PCC dashboard and UDA tab and other significant reports for the following items and follow-up with by Charge Nurses/RPNs for non-compliance; report repeat offenders to DOC.
