- Assist Patients of Daily living. This includes feeding, lifts and transfers, skin care , oral hygiene, and toileting.
- Performs tasks delegated by RN or RPN, this include actions that involve insertion of a digit or instrument into a body.
- Reported concerns to nurse supervisor to promote optimal care.
- Communicate and demonstrate basic information in accordance with patients pre- established Plan of Care.
- Ensure a safe environment for patients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavior, physical, or cognitive changes. Safety concerns reported to RN or RPN and other regulated health provider.
- Made beds, sanitized surfaces to support activities of daily living.
- Complete and maintain related patient records and progress notes, as per hospital policy.
- Documented vitals, behaviors, and medications in client medical records.
- Assisted with feeding and monitored intake to help patients achieve nutritional objectives.
- Assisted with dressing guidance, grooming, meal preparation, and medication reminders.
- Consulted with supervisors to assess cases and plan strategies for enhancing care.
- Assisted clients with daily living needs to maintain self-esteem and general wellness.
- Provided safe mobility support to help patients move around personal and public spaces.
- Monitored client vital signs, administered medications, and tracked behaviors to keep healthcare supervisor well-informed.
- Assisted patients with bathing, grooming, dressing, and oral hygiene care both in private residences and facilities.
- Maintained strong connections within caregiver community to gain continuous knowledge.
