Assist clients to identify, build, and maintain a support system
Facilitate connections to needed/wanted services, supports, and resources, including those in the broader community

Provide a continuum of integrated intensive case management (ICM) and Assertive Community Treatment (ACT) services required by clients with complex care needs in the Mid-East Toronto Health Link
Conduct face-to-face assessments with clients at their locations including but not limited to home, streets, shelters, hospitals, and detention centers
Work collaboratively with physicians, referral source, housing services, probation officers, families, ODSP, OPGT, and other stakeholders in client care to ensure a well-coordinated and effective service system
Monitor a 24/7 crisis phone line and provide supportive counselling to clients
Effectively provide comprehensive support to clients with chronic mental illness and addictions with issues ranging from therapeutic, housing, vocational, housing, medication and symptom monitoring
Contribute to the development of comprehensive client-centered assessment and treatment plans, as well as Community Treatment Orders, complete Ontario Common Assessment of Needs (OCAN)
Assume responsibility for coordinating and facilitating therapeutic and recreational client group sessions.
