Projects

Merger of two outpatient dialysis clinics as delegated by Senior Leadership. Replacement of one Erythropoiesis-Stimulating Agent (ESA) in the outpatient dialysis clinic to a more cost-effective ESA that can be administered less frequently, improving productivity, costs, and patient satisfaction. Oversaw the opening of a new inpatient dialysis unit at one of the hospitals. Developed an audit system for monitoring patient outcomes, staff compliance, and more in order to be survey-ready at all times. Led a team of leaders in the development of a professional partnership between Community Health System and a regional healthcare company to develop a Transitional Care Clinic. The purpose of the clinic is to provide continuity of care to underserved patient populations, i.e., uninsured patients and repeat admissions, by connecting discharged patients with a provider that will support post-admission needs. This program supports patient throughput, has reduced 30-day patient readmissions, and helps to ensure patients leave the hospital with a quality follow-up plan of care. Supported the partnership between Community Health System and a local hospice agency in the development of a General Inpatient Program (GIP). The purpose of this partnered program is to allow patients going on hospice or comfort care to transition to hospice within the hospital while still being under the care of the local hospice agency.
