· Develop a positive, supportive and cooperative relationship with Core Members through sharing daily life together, which respects their Personal Value Statements, history, preferences, choices, personal goals and needs.

· Ensure, in collaboration with other team members, that the physical, emotional, spiritual, vocational, recreational, health and safety needs of Core Members are met while maximizing their choice and participation in the decisions affecting their lives.

· Participate in the facilitation, design and implementation of the Person Centered Plan for Core Members as directed by the Home Coordinator.

· Take daily action in supporting the Core Members in their attainment of their personal goals and values as outlined in their Personal Value Statements and their Person Centered Plans.

· Respond to each Core Member’s individual needs by respectfully supporting them with their personal hygiene, care, dress, eating, etc., as needed, in a manner that enhances their dignity and appropriate appearance.

· Support and honour all recommendations in the core member’s care plans

· Encourage and support Core Members in the skills and gifts needed to promote their personal growth and accordance to their care plans, and to contribute actively to their home life, Shekinah community and neighborhood.

· Accompany and support Core Members to personal and medical appointments, and to other activities outside the home which enhance the social, recreational, educational and spiritual aspects of their lives.

· Administer medication as prescribed by a physician and adhere to all policies, procedures and regulatory requirements.

· Adhere to all procedures, protocols and Shekinah policies.

· Play an active role in developing supportive relationships with professionals such as medical professionals, facilitators, community inclusion workers, counselors, teachers,ministers, etc.

· Encourage each core member to develop and nurture friendships within and outside the community, developing networks of support.

· Report concerns regarding the core members to the Home Coordinator in a timely manner.

• Assisting nursing staff in the care of all patients including bathing, changing diapers and feeding
• Listening to the nurse instruction-drug support
• Providing quality patient care
• Managing confidential files
• Performing simple procedures like wound dressing
• Changing positions of bed ridden patients, alone or with assistance to prevent bedsores
• Taking and recording vital signs and intake and output as directed
• Cleaning, sterilizing, storing, preparing, dressing packs and other supplies
• Cleaning and maintaining healthy and safety environment
• Transferring of patients from bed to wheelchair and vice versa
• May be assigned to specific area of nursing home, or medical facility
• May assist nursing staff in care of geriatric patients
