• Provides comprehensive care to the patients as part of an interdisciplinary/inter
professional team focused on creation of a person centered plan of care.
• Conduct patient assessments using the RAl·MDS tool
• Run and review reports from the Learning Management System, auditing of clinical
compliance reviews and documentation related to patient care.
• Working in collaboration with the Patient Flow Coordinator, assist with incoming patient
admissions by ensuring pharmacy services, medical equipment and supplies are
available and any last-minute discharges are communicated. Send the morning report to the Patient Flow Coordinator(s).
• Participate in quality activities and continuous improvement initiatives in keeping with
the company's Quality Management System. Participate in daily huddles and weekly
quality huddles.
• Responsible for immediately completing Accident Reports via RL6 for all direct reports who injure themselves on the job, and monitor and support employees participating in modified work programs.
• Identifies and implement appropriate clinical documentation and record keeping
practices.
• Participates in ongoing quality monitoring and improvement.
