Service Responsibilities:

• Provide ongoing case management and advocacy for children and youth involved with Child Welfare agencies, while ensuring culturally appropriate, child-centered support.
• Arranging, facilitating, and attending case conferences, sharing circles, and placement meetings with children, families, Elders, and service partners.
• Conducting home/foster home visits with Child Protection workers; monitoring safety plans and support least-intrusive measures to maintain child safety & stability.
• Maintaining consistent communication among First Nation, Elders, families, and Child Welfare agencies to effectively coordinate services.

Children & Youth Case Management Responsibilities:

• Tracking and documenting all child/family involvement, maintaining comprehensive files including Plans of Care, legal agreements, court documents, and consents.
• Preparing accurate case notes and correspondence.
• Attending ongoing case conferences to support families and advocate for children’s best interests.
• Monitoring placements and visitation to ensure regular contact with children and families.
• Coordinating access to culturally appropriate services, prevention programs, and community resources for children and families.
• Providing education and awareness on child welfare processes and the Mamow Wee Chi Ti Win Band Representative Program.

First Nation Responsibilities:

Ensuring Child Safety:

• Conducted home visits & investigations in high-risk and crisis situations while assessing safety, gathering information, and making evidence-based decisions.
• Conducted interviews with clients, families, witnesses, and community professionals to gather information, assess credibility, and support case planning.
• Developed and monitored individualized intervention and risk management plans while coordinating appropriate support and services.

Documentation:

• Prepared comprehensive case notes, investigative reports, assessments, and documentation while maintaining accuracy, objectivity, and confidentiality.

Collaboration & Liaising:
