Engage with clients on a one-on-one basis, observe and deliver medications, meet the needs and concerns of clients, provide necessary and unique care to each client, meet clients at their residences or in the community, ensure their safety, provide counseling and activities when needed, maintain good rapport and confidentiality with each client.

• Assist service users with accessible applications, suitable housing and move-in processes.
• Extensive experience with note taking and recording.
• Performing wellness checks by distributing harm reduction equipment.
• Practice confidentiality discussion with service users.
• Well versed in compassion and supportive allyship.
• Recognize turning points for service users wanting to make changes
• Clear understanding of duty to report and other policies and procedures implemented by BFN

• Provide prompt, efficient service and suggestions for customers
• Managed time efficiently to complete all tasks within shift.
• Trained and coached team members to maintain busines standards
• Proven ability to learn quickly and adapt to new situations.

• In partnership with program participants, establish individualized care plans for integration into available housing.
• Provide supportive information on trauma, mental health and harm reduction.
• Ability to work with individuals with diverse and complex health and psycho-social needs
• Participate in complex care planning meetings and case conferences involving relevant clinical and community supports.
• Accompaniment to community-based appointments for service users
