Physically fit; able to perform lifts and transfers ‘G’ license with reliable transportation Qualifications Experience working in long-term care, residential and hospice settings Providing person centered care to patients with dementia and cognitive impairment Training of Personal Support Workers, Nurses and supervise staff Excellent nurse-patient therapeutic relationship skills; support patients with Activities of Daily Living, Collaborate with physicians, physiotherapists, psychologists and occupational therapists Assist with developing patient care plans including assessments, evaluations and nursing diagnoses Counsel and educate patients about adjusting to their limitations, health maintenance and disease management Ensure efficacy of treatments through monitoring of treatment regimens, Demonstrate competence in the performance of the following skills: ECG, arrhythmia interpretation, IV therapy (including maintenance, IV starts and IV medication administration), splinting, oral IM and Subcutaneous medication administration, wound care, CBGM and phlebotomy, COPD, pneumonia, stroke relapse, and palliative care Accurately perform lab tests including C&S, urinalysis and urine pH, perform catheter insertion and discontinuation and follow infection control procedures Manage and care for ostomy and colostomy patients and effectively monitor fluid intake and output levels Successful completion of Comprehensive Advanced Palliative Care Education (CAPCE) Successful completion of Wound Care and Foot Care Experience with medical conditions including Parkinson's, Dementia, Diabetes, Cancer, Alzheimer's and MS Schedule patient admissions and discharges and provide emotional support to patients and their families Completion of the Manager Training Program Organized and dependable candidate successful at managing multiple priorities with a positive attitude.
