Under contract with the VA, I supported homeless veterans in Mendocino County to find and maintain permanent housing. I liaised with various community entities and landlords and met regularly with veterans to support case management and therapeutic needs. I maintained a caseload of over 35 veterans, all with complex vulnerabilities and many with severe mental health diagnoses. I abided by evolving HUD and VA regulations, and regularly performed myriad mental health assessment measures for each veteran on a regular basis.
I maintained a community-based clinical caseload of high-risk transition aged youth with severe, persistent and pervasive mental health symptoms and their families, managing crises and collaborating with other clinicians and case managers to coordinate care. I regularly billed for 27.5 hours per 40-hour week and was fully familiarized and compliant with Medi-Cal documentation standards. I attended weekly 2-hour case conferences, 2 hours of individual supervision weekly and held frequent clinical team meetings toward best treatment planning practices.
As Admissions Coordinator of this 24-hour, 13-bed residential facility providing services to transition-aged youth with severe and persistent mental illness, I managed the entirety of the referral and admissions process; this included: outreach, prescreening and assessing referred clients in the community or at Casa and subsequently advocating for clients as clinically appropriate, documenting contacts and building relationships with providers and referral sources. Provided a welcoming environment for visiting providers as well as new and visiting clients. I ensured episode and chart opening documentation standards. I maintained close and regular contact with referral/providing teams from client’s referral through their admission and worked closely with clients to assure smooth transition into the program. Maintained a caseload of 2 residential clients, coordinating regular individual sessions, team meetings and providing clinical support as needed. Attended weekly group supervision.
As Lead Clinician, in collaboration with and under the supervision of the Program Director, I served our clients by providing clinical and managerial support to five full-time clinicians and three relief staff. I was on-call to support clinicians as they managed crises and to cover open shifts. I led groups and took on caseloads of clinicians when needed, and provided overarching clinical support to the team. I ran weekly process groups and house meetings, completed Medi-Cal billing and oversaw quality assurance of documentation standards as set forth by Medi-Cal, Alameda County and Community Care Licensing, supporting clinican compliance as appropriate. As a part of the counseling team at this Level-12 charter school, I conducted weekly individual and group play, art and talk therapy with a caseload of 15 special education students ages 8-18, the majority of whom had Emotional Disturbance diagnoses. Led weekly school-wide treatment team training meetings; interfaced with teachers, staff, parents, school districts, state agencies and law enforcement as needed to coordinate care. In the capacity of MFT Intern I maintained a caseload of 15-20 demographically and diagnostically diverse adult clients at sliding scale community mental health clinic. Attended and presented cases in weekly group supervision.
In the capacity of Assistant Director I led several staff groups each week, providing training and executive direction to all staff in the Institute; managed staff and developed protocol for high-volume phone shifts; developed and implemented clinical and administrative training programs for MFT trainees and interns; interviewed and selected candidates for traineeship and internship positions; provided broad administrative support to the Director; collaborated to create accessible yoga programs for clients and community.
