PHN works with Pregnant Clients and New Mothers. Home Visits are made on a monthly basis. Physically assess pregnant clients especially with weight and blood pressure. Teach them about fetal development , discuss importance of lab test, ultrasounds, and immunizations as well as early and consistent care during pregnancy. Post Partum, PHN makes sure MOB follows up for her postpartum exam and has information on types of birth control . The importance of breast feeding is discussed and PHN connects MOB to lactation Specialists. After birth, PHN will assess infant health, weight checks growth and development, milestones, infant safety especially involving safe sleep, feeding and nutrition, immunizations, and car seat check ups. PHN also connects MOB and family to insurance and community resources. Children and Youth with Special Health Care Needs receive additional support.
