•I was rotated in General Ward last Oct 2022 for a week. My responsibility was to monitor the patients vital signs and ensure the patients need are met.

• I came back to have duty on Dec 2022 in Outpatient Department. I communicated with doctors and nurses about patients charts and write the clients health concerns for doctors reference in the OPD.

•I was assigned to work in Outpatient Department. I assessed the clients health. Gather their vital signs for their health history and performed wound cleaning.

• Provide direct care to the individual in the community through home visits

• Participated in safety and security for children in Daycare in Brgy. Sta. Rosa Iguig, wherein we educated and teaches them the things they should know for their safety and for their health also.

• Participated in Educating the community in Brgy. San Esteban about preventive health care and promote self care.

• Assessed patient health history and performed nursing skills such as giving vaccinations.

• Obtained knowledge on providing basic care for patients through 3 hours of classroom time.

•I was rotated in General ward, I am assigned as a bedside student nurse. My duty was to monitor the patient, charted the patient vital signs and provide nursing care.

• On October, I was rotated in Dialysis Department. I performed how to set up correctly the dialysis machines and assessing the health of the patients throughout their treatments. and priming the dialyser and administer medications.

• Assisted care to the clients including administering medication, obtaining and recording vital signs.

• Assisting with the labour and delivery process and providing care for the infant and mother

• Giving education about parental for first time mothers

• Assigned in Surgery ward as a team leader for my group and I take care for all of the patients charts and monitoring the members duties.

• Giving the patients nursing care such as post operative care and giving medications on assigned patient.

•Helping patients for their pre operative care

• Assigned in Cardiology ward as a medication nurse. I prepared and administer medications.

•Obtained new skills such as EKGs and assisting life support.

• Assigned in Gastrointestinal Ward. Helped improve patient outcomes by educating and advising on relevant treatments and care.
• Explained procedures to patients to reduce anxieties and increase patient cooperation.
• Developed strong rapport with patients while maintaining appropriate professional boundaries.
• Assigned in Oncology Ward and performed basic care to the patients
• Collected and documented patient medical information such as blood pressure and glucose level.

• Obtained and documented patient medical history, vital signs and current complaints at intake.
• Obtained skills how to the balanced skeletal traction
• Learned about traction, cast, mold, brace and orthopedic hardware
