In my role as a Health Care Social Worker, my role and responsibilities include providing a diverse range of interventions to patients enabling them to achieve their optimal level of functioning and health.

Interventions included, but were not limited to:

• psychosocial assessments
• adjustment therapy
• supportive therapy
• crisis counselling
• resource counselling
• discharge planning
• case management
• advocacy
• education
• program planning
• consultation and collaboration

My role also requires me to work autonomously as the only social worker supporting the unique needs of two hospital sites, as well as working collaboratively within multiple interdisciplinary teams within the sites.

In the context of my role, I regularly utilized a variety of therapeutic modalities, including, but not limited to Motivational Interviewing, Solutions Focused Therapy, Mindfulness and Cognitive Behavioural Therapy.

With my role as a Clinical Therapist, I worked as a part of the multi-disciplinary team to provide comprehensive services to adults with mental health/addiction disorders.

Within this role, my responsibilities included the following:

• conducting comprehensive psycho-social assessments
• treatment planning
• provision of professional clinical services for clients living with complex mental health disorders and/or concurrent disorder(s)

As a Social Worker, my role was to work as a part of a multi-disciplinary team towards the goal of protecting children from abuse or neglect and providing ongoing services to parents, and children.

As part of this role, my responsibilities included, but were not limited to:

• Practicing with a specialized level of knowledge, skills, and training towards supporting the health, safety, and well-being of children under the care of the child welfare agency as well as those who remain in the care of their parents where concerns existed
• Demonstrating the ability to actively engage parents and families to address challenges and trauma, strengthen relationships, and create positive change
• Conducting timely, complex, and comprehensive investigations of child protection concerns
• Providing protection services to children and families to ensure their safety and well-being
• Preparing legal documentation for court hearings, settlements, and participating in legal case conferences as well as testifying in court proceedings
• Planning, developing and monitoring of case plans with families with diverse needs
• Participating in risk assessments, clinical evaluations
• Supervise and provide services to children in the Agency's care
• Maintain records on the case management system
• Counsel individuals and families
• After-hours emergency duty on a rotational basis
• Working closely with community and advocating for connections to community resources
• Facilitate, support and monitor adoption placements

Under the supervision of a Clinical Therapist, I worked as a part of the multi-disciplinary team to provide comprehensive services to adults with mental health/addiction disorders.

Within this role, my responsibilities included the following:

In addition, I worked with a physician to support the Opioid Replacement Treatment Program. Within this role, my responsibilities included:

• resource counselling
• therapeutic clinical interventions for clients dealing with complex addiction issues
• treatment planning

As a Social Worker, my role was to develop plans with children and families to ensure the safety of children through assessment and collaboration. Within this role, my duties included:

• Receiving, assessing and responding to concerns of child abuse and neglect by interviewing parents and children from a strengths-based perspective
• Making day-to-day guardianship decisions for a child throughout a child’s stay in care and developing a plan of care with the child and involved others
• Preparing legal documentation for court hearings, settlements, and participating in legal case conferences as well as testifying in court proceedings
• Planning, developing and monitoring of case plans with families with diverse needs
• Participating in collaborative risk assessments on an ongoing basis with respect to the goal of reunification of children and their families
• Maintain records on the case management system
• Counsel children and families on an ongoing basis
• Working closely with community resources
• Facilitate, support and monitor adoption placements
