Tracked clinical documents to maintain compliance, assisted scheduling & coordination of services and clinical disciplines as needed, scheduled clinicians to provide appropriate care, followed up regarding missed visits, maintained on call/weekend calendar, contacted clinicians regarding pending documentation and late initiation notes, called patients and conducted patient care surveys. Tracked caseloads and frequencies of all clinicians, uploaded documentation to patient’s charts (ex. lab orders, IV orders, additional MD orders, etc.), obtained weekly schedules from clinicians and kept them on file, obtained lab results and INR results; sent them to clinicians, uploaded to charts, and faxed to MD, was involved in case conference meetings, etc.
