Case Management:

• Foster participant engagement and motivation, conducting assessments to identify service needs in the areas of behavioral health and housing, de-escalating behavioral health crises as needed.

• Individualized service plan development and continuous updating, tracking and documenting participant progress against the plan, developing new relationships with outside resources to meet participant needs, and eliciting participant feedback regularly regarding service needs and satisfaction.

Service Coordination and Leadership:

• Organizing and facilitating case conferences or other appropriate communication among all team members regarding participant progress and service needs.

• Transmitting information from and about participant to other team members accurately and in a timely manner, and ensuring participants access needed services from Rubicon’s financial, legal, career and wellness teams and with community partners and coordinating multi-disciplinary services overall.

Monitor Contractual Goals:

• Support achievement of the programs outcome goals, productivity targets, and all other performance metrics

• Support reporting; progress against program goals & objectives.

Case Management

• Provide information or refer individuals to public or private agencies or community services for assistance and keeping records or prepare reports for owner or management concerning visits with clients.

• Advocate for individual or community health needs with government agencies or health service providers by attending community meetings or health fairs to understand community issues or build relationships with community members.

• Contact clients in person, by phone, or in writing to ensure they have completed required or recommended actions. To identify or contact members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or pregnant women.

• Identify the particular health care needs of individuals in a community or target area and interpret, translate, or provide cultural mediation related to health services or information for community members.

Outreach & Advocacy

• Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community. Maintain updated client records with plans, notes, appropriate forms, or related information.

• Provide feedback to health service providers regarding improving service accessibility or acceptability. Refer community members to needed health services. Assist families to apply for social services, including Medicaid or Women, Infants, and Children (WIC).

• Collect information from individuals to compile vital statistics about the general health of community members. Develop plans or formal contracts for individuals, families, or community groups to improve overall health. Report incidences of child or elder abuse, neglect, or threats of harm to authorities, as required.

• Transport or accompany clients to scheduled health appointments or referral sites. Advise clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services.

Outreach & Advocacy

• Advocated on behalf of homeless clients with housing resources and built relationships with a variety of housing program officials.
• Successfully secured housing for homeless individuals in a timely manner.
• Assisted indigent and systematically impacted clients as well as persistently mentally ill clients in identifying and accessing Housing Resources

Housing Specialist

• Assisted clients in completing housing applications with individualized housing plans while finding placement for clients in transitional housing.
• Getting approval and securing permanent housing by deadlines.

Threat Assessment

• Conduct routine threat assessments before and during a security detail while providing close protection security for principal/protectee.

• Fluid in extraction, evacuation and evasive driving.

Cyber Security

• Managing databases and logbooks of information gathered during threat assessment and advancing techniques.

• Establish and maintain healthy relationships and communication with local law enforcement leads for protocols, procedures and rules of engagement

Counseling

• Provide direct counseling services, including individual and group counseling sessions, treatment assessment and planning and case management as needed. Administers therapeutic treatment such as behavior modification and stress management therapy to assist patient in controlling disorders and other problems.

• Changes method and degree of counseling when indicated by client reactions. Discusses progress toward treatment goals with clients.

Intake Coordinator

• Maintain excellent client records and complete all agency required reports and paperwork.

• Refers client to supportive services to supplement treatment and counseling.
