• Completes case management
• Work with team members to provide comprehensive support, ensuring individuals with disabilities can achieve their goals.

• Completes medical appointments for the Individuals
• Plans and moderates yearly Individual Journey Meetings where Individuals Service Plans are set (ISP's)
• Completes Relationship-based and annual supervision summaries for team members
• Ensures team members are up to date with mandatory and required training
