Provides individualized care to clients/families by initiating discussions with client/family to identify what is important from the perspectives of the client/family, assessing client health status, identifying client/family priorities and plans and discussing professional care and services that support these plans. Provides a variety of health care interventions utilizing nursing skills and applicable medical equipment.

4. Establishes therapeutic relationships and provides a safe environment to facilitate open, honest and confidential discussions with clients/families. Provides emotional support to clients by identifying what life is like and understanding their concerns, wishes and goals, learning needs and readiness and ability to learn. Advocates for, and facilitates informed choices. Teaches clients/families about management and treatment of specific medical conditions and their symptoms including demonstrating self/home care skills.

5. Provides counselling and education and acts as a resource on health information for patients/clients and families regarding illness, injury, results of various tests and overall health promotion and wellness. 6. Assesses patients/clients for discharge requirements at time of admission and implements discharge plans as established by interdisciplinary team by liaising with other team members regarding patient’s/client’s status/care needs and/or identifying referrals to other health care professionals and/or arranging for access to other resources such as respite beds, additional home support services and home care as needed. Provides education to patient/clients/families on discharge. 7. Documents client/family perspectives, nursing observations and activities that support patient/client/family priorities. Maintains complete and accurate computerized and paper records by inputting data and documenting client information in accordance with established procedures and policies. 8. Participates in unit/team/program activities required to enhance and optimize the delivery of care and to facilitate evidence informed nursing practice. Integrates quality improvement principles and activities into nursing practice. Collaborates on quality improvement activities with the inter-professional/interdisciplinary team, inclusive of patients/clients and families. Participates in the development and review of policies, procedures and guidelines as required. Provides input/recommendations to Manager and/or other team members regarding efficiencies of processes/procedures in relation to work duties. 9. Follows agency protocols and guidelines for practice to enact evidence informed practice. Accesses digital formats of clinical practice guidelines and agency-approved protocols. Evaluates results in collaboration with interdisciplinary team.

10. Participates on committees as directed to promote optimal support for clients, to facilitate the resolution of issues and to advocate for the goals and objectives of the unit/team/program. 11. Finds and reads research reports relevant to own practice area. Identifies credible and reliable sources of knowledge on which to build practice decisions relevant to own care delivery setting (e.g. publications, library, online information, knowledgeable professionals). Integrates research findings with clinical expertise and patient needs for optimal care. Collaborates effectively with team of researchers. 12. Participates in approved research and special projects in collaboration with designated personnel by collecting data, summarizing information and/or providing updates for review and further analysis by principal researcher, research team and/or project coordinator. 13. Demonstrates and/or provides guidance on clinical and health related tasks to designated care givers and guides the clinical experience of assigned students and new staff. Provides input to the Manager and/or designated as requested on individual's progress in achieving established standards of nursing care. 14. Carries out responsibilities in accordance with WorkSafe BC and organizational Occupational Health and Safety policies and standards to meet safety requirements. Participates and remains current in mandatory education, such as infection control, violence prevention and protection of privacy/confidentiality. Page: 3 15. Immediately reports unsafe situations by notifying supervisor or other appropriate personnel and using online safety learning system (SLS). Shares learnings from safety events with team members and incorporates recommendations within clinical practice to enhance patient/client safety 16. Sets measurable personal performance goals in collaboration with the Manager for professional development such as updating own knowledge within area of practice by reviewing current literature and research, identifying new theories, trends and opportunities and developing a plan in collaboration with the Manager for professional development. Reviews progress to ensure that goals are achieved within established time frames. 17. Performs other related duties as required.

: I worked as a senior sister in the Interventional Radiology Department. My clinical duties are to admitting the patient for procedure ,scrub for the procedures, work as sedation nurse and to recover the patients post procedure. Communicate with the multidisciplinary team to arrange the daily procedure list after evaluating the risk involved in the procedure. I always ensure that the patient receive the high standard of care according to the local and national safety standards.

Managerial responsibilities: Work as a team leader, co-ordinate the list of the procedures to ensure the list runs effectively and in safe manner. Undertake challenging conversation with the staff and the patient effectively to resolve conflicts always maintaining privacy and confidentiality.
