Responsible for other units in the hospital after hours and on weekends.

• Advocate for patients' rights ensuring they receive appropriate services and supports needed.
• Maintain confidential case documentation with Connect Care.
• Plan and coordinate discharge from hospital.
• Facilitate referrals to appropriate community resources based on patient need.
• Provide end-of-life counseling and support to patients and their families.
• Provide crisis intervention and emotional support to patients and their families.
• Conduct psychosocial assessments of individuals seeking services in the ED.
• Assess risk and report to authorities when required (domestic violence, child protection, elder abuse etc)
