Responsible for leading an operational team in the planning, delivery and evaluation for Family Medicine Programs and services in support of the vision, mission and business plan of the Department of Family Medicine.
  •  Responsible for developing and maintaining relationships with local and provincial representatives and healthcare stakeholders.
  • Supported the implementation of Wave 2 of Connect Care for the physicians at two urgent care sites. Responsible for attending meetings with stakeholders, educators, physician leads, and managers. Provided reports and as coordinated with AHS Department Head.
  •  Ensures there is efficient, effective and equitable allocation of resources (people and financial) within the Department.
  •  As a member of the Family Medicine Senior executive team, accountable for the development, implantation and evaluation of overall strategic and operational plans to support patient and family focused care programs and delivery. The “Patient Medical Home Model” is a strategic vision for the primary care clinics. A place where a patient can receive care from a team that knows their history, their family, and from care providers that they have developed relationships with. This continuity of care has shown to reduce costs, promotes efficiencies, improve patient outcomes and reduce visits to urgent care and emergency departments.

This award highlights important work being done to improve access to effective mental health care, enhance mental health service delivery, and increase family physicians' capacity to identify and meet the mental health care needs of their patients.
