Summary

Social workers engage clients and stakeholders to work on common goals to improve client independence, level of functioning and connections to their community. Social workers are the initial point of agency; they take responsibility for screening calls and determining needed services/supports to which clients (and their families) can be referred. In addition, social workers are involved in community development work, support groups of Child and Youth Worker and collaboration and partnership initiatives.


Child and Youth Workers support students in skill development, self management and problem solving to improve academic achievement. Programs are developed to meet the individual needs of each student. develop and provide individual behavior management programs for students in consultation with the classroom teacher provide assistance for students in the acquisition, reinforcement and maintenance of academic, communication and social skill providing services as part of a multidisciplinary team can be assigned to individual students They work in collaboration with the multidisciplinary team in planning, implementation and evaluation of treatment care and transition plans.


Responsibilities:


Develops supports catered to individuals based upon Independent Support Plans/Behavior Support Plans and directs staff to implement support to achieve goals, involving families and community supports Provide mental health support to seniors through effective assessment. Support to social worker, developer social worker in returning home or to an alternate setting by watching over children's and addiction support. Screen and assess clients’ eligibility for Care services; provide consultation and counseling regarding the appropriateness of services. Working collaboratively with children in school and community providers to establish a coordinated care plan based on clients physical, mental health and cognitive needs.

EDUCATION AND EXPERIENCE


1-2 years graduate experience with children and youth who are experiencing a mental health issue.
Ability to provide individual, and good therapy.
Experience with mental illness and high-risk children and adolescents.
Demonstrated ability to assess children and youth in crisis.
Experience with behavioral programming for children and youth.
Demonstrated excellence in problem solving techniques, conflict resolution, and flexibility.
Co-operative relations within an interdisciplinary team.
Ability to communicate effectively.
Knowledge of Community Resources for Child and Adolescent mental health.
Demonstrated interest in continuing education.
