The TDOS Worker will provide frontline peer support, advocacy, and navigation assistance to individuals
accessing mental health, harm reduction, and addiction services. This role will also support intensive case
management activities, peer outreach services, and referrals to detox, treatment, and Opioid Agonist
Therapy (OAT) programs.

Meet, transport, and attend client appointments (medical, housing, professional, etc.) as
required, while also visiting or connecting with clients transitioning from hospital care to ensure a
smooth reintegration into the community.

Provide crisis intervention when clients are in distress and ensure ongoing follow-up by
connecting them with Ltmhs programs and various agencies to complete necessary tasks.

Attend regular meetings to discuss client concerns and wellness plans, provide timely reports,
and advocate for clients when appropriate.
Participate in educational presentations to the public and other stakeholders while also
contributing to improving system performance by identifying and addressing gaps, overlaps, and
redundancies.

Conduct client screenings and assessments during admission and non-admission.
Complete admissions and discharges; perform related administrative tasks.
Perform 24-hour assessments and record vital signs including blood pressure, pulse, and
respiration. Accurately document client information, including discharge summaries, in a timely
manner.
Assist in crisis situations requiring medical or behavioral intervention.
Deliver client education, including Take Home Naloxone training.
Facilitate house meetings and recovery planning in support of client self-determination.
Coordinate support with the inpatient discharge coordinator.
Perform housekeeping tasks, including laundry and general cleaning of kitchen and
common areas.
Carry out additional duties as directed by the detox supervisor or designate.

• The shelter provides basic needs and supports to the homeless, at risk, and street population. My role is the intake of the individual into the shelter, obtain basic personal information and work collaboratively with individuals to provide 24 hour support. The shelter worker provides individuals with food, basic personal hygiene products as well as crisis and life skills support. Effective team player who is honest, non-judgmental, and non-intrusive; Experienced and trained to work with those who have mental health issues, substance misuse, poverty and trauma; knowledge and application of client centered theories and approaches; good interpersonal and communication skills; knowledge of community resources; ability to work co-operatively with co-workers, supervisors and other professionals; strong ability to problem-solve and make ethical decisions.
