
Experienced with managing outreach programs and building community partnerships. Utilizes strong communication skills to engage diverse groups and foster collaboration. Track record of enhancing community support and driving successful outreach initiatives.
The TDOS Worker will provide frontline peer support, advocacy, and navigation assistance to individuals
accessing mental health, harm reduction, and addiction services. This role will also support intensive case
management activities, peer outreach services, and referrals to detox, treatment, and Opioid Agonist
Therapy (OAT) programs.
Meet, transport, and attend client appointments (medical, housing, professional, etc.) as
required, while also visiting or connecting with clients transitioning from hospital care to ensure a
smooth reintegration into the community.
Provide crisis intervention when clients are in distress and ensure ongoing follow-up by
connecting them with Ltmhs programs and various agencies to complete necessary tasks.
Attend regular meetings to discuss client concerns and wellness plans, provide timely reports,
and advocate for clients when appropriate.
Participate in educational presentations to the public and other stakeholders while also
contributing to improving system performance by identifying and addressing gaps, overlaps, and
redundancies.
Conduct client screenings and assessments during admission and non-admission.
Complete admissions and discharges; perform related administrative tasks.
Perform 24-hour assessments and record vital signs including blood pressure, pulse, and
respiration. Accurately document client information, including discharge summaries, in a timely
manner.
Assist in crisis situations requiring medical or behavioral intervention.
Deliver client education, including Take Home Naloxone training.
Facilitate house meetings and recovery planning in support of client self-determination.
Coordinate support with the inpatient discharge coordinator.
Perform housekeeping tasks, including laundry and general cleaning of kitchen and
common areas.
Carry out additional duties as directed by the detox supervisor or designate.
