Clinical pathway operations

• To implement the clinical pathways as decided by the clinical pathways team, under the direction of the unit head.
• Ensure effective on-boarding of patients under the guidance of the unit head. ensure the service components are provided to the patients as per the protocols,
• Medical record documentation, vital monitoring and escalation of issues to the treating team give inputs to the product team under the guidance of the unit head
  work in the close coordination with the quality improvement team for accreditation, quality improvement programs and incident analysis and corrective action
• Work in close coordination with the other services verticals like Physiotherapist, dietetics and Psychologist, etc.
  any other work that may be assigned by the supervisor from time to time.

• Evaluate and assesse nutrition status of patients, and screen patient's for nutritional risk in accordance with established department protocols and standards.
• Plan and initiate medical nutrition therapy to patients if indicated by patient requirements
  and clinical protocol.
• Educate and counsel patients and families on nutritional issues, either individually or in groups.
  Communicates individual nutritional care plans to other health care providers as a member of a health care team.
• Order and maintain inventory of program supplies, including patient checks, for all clinics.
• Perform various administrative duties related to public health nutrition programs.
• Maintain professional growth and development through completion of required departmental competencies.
• Perform miscellaneous job-related duties as assigned.
• Make diets according to disease condition and weight loss
• Manage the appointment of the clients and deal alone.
• Worked as a Centralized diet consultant and supervise 3 clinics at a time.

• Deep knowledge about diet and disease.

• Monitor exact, detailed reports and records of the critical ICU patients.
• Monitor and record symptoms and changes in patients' conditions and information to the physician’s order, interpret and evaluate diagnostics tests to identify and assess the patient's condition.
• Carefully observe and document patient medical information and vital signs.
• Document patients' medical histories and assessment findings.
• Document patients' treatment plans, interventions, outcomes, or plan revisions.
• Consult and coordinate with health care team members about whole patient care plans.
• Modify patient treatment plans as indicated by patient's response and conditions.
• Monitor the critical patients for changes in status and indications of conditions such as sepsis
  or shock and institute appropriate interventions.
• Administering intravenous fluids and medications as per doctor's order.
• Monitor patients' fluid intake and output to detect emerging problems such as fluid and electrolyte imbalances.
• Monitor all aspects of patient care, including diet and physical activity.
• Identify patients who are at risk of complications due to nutritional status.
• Direct and supervise less-skilled nursing/health care personal or supervise a particular unit
  on one shift to patient's response and conditions.
• Treating wounds and providing advanced life support.
• Assist physicians with procedures such as bronchoscopy, endoscopy, endotracheal intubation, Tracheostomy and elective cardioversion.
• Ensuring that ventilators, monitors, and other types of medical equipment function properly.
• Ensure that equipment or devices are properly stored after use.
• Identify malfunctioning equipment or devices.
• Collaborating with fellow members of the critical care team.
• Responding to life-saving situations, using nursing standards and protocols for treatment.
• Also perform administrative duties.
• Assess patients' pain levels and sedation requirements.
• Prioritize nursing care for assigned critically ill patients based on assessment data and identified needs.
• Assess family adaptation levels and coping skills to determine whether intervention is needed.
• Act as a patient advocate.
• Providing education and support to patient families.
• Able to draw ABG Blood and interpret the report correctly.
• Have enough knowledge about GCS (Glasgow Coma Scale) and also evaluate capacity of patient condition.

• Diagnose health problems by reviewing patients' health and medical histories, questioning, observing, and examining patients and interpreting x-rays.
  Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
• Monitor, record, and report symptoms or changes in patients' conditions.
