Personal caseload of 40+ clients. Includes referrals for varied healthcare related needs such as resource advocacy, navigation of complex family dynamics, financial supports, and investigations of reported abuse and neglect under The Adult Guardianship Act.

Collaboration among care teams and community resources to support adults in directing their own goals and maintaining quality of life in a way that respects their rights and autonomy. Working knowledge of legislation related to Adult Guardianship, as well as consent and future planning.

• Registered Social Worker within a multidisciplinary Community Outreach team. Supporting predominantly unhoused individuals with ongoing substance use who may experience barriers to care. Case load included clients with longer term goals, as well as those requesting support in-the moment on an ambulatory basis. Ongoing collaboration with community services and residential treatment centres BC Wide to support client-centered care plans and connect clients to services most appropriate for their goals of care.

• Point of contact for Ryan Hill Apartment Program, a 13-unit residential program which utilizes the principals of psychosocial rehabilitation to support program participants in working towards their stated goals for independent living. Position included planning, development, implementation, and delivery of support services as well as overseeing the day to day operation of the program. In this position I acted as a liaison with community agencies; promoting and supporting community involvement/collaboration with the program to ensure that clients were supported in their stated goals. Other duties include the supervision of regular staff members as well as a pool of casual staff. Creating statistical reports and completing administrative tasks as required.

• Full-time employment with Ryan Hill Apartment Program. Worked directly with 13 residents whose mental health and/or substance use have impacted their ability to live independently. Utilized the concepts of psychosocial rehabilitation to support residents in housing skills and other goals, as outlined in individualized care plans.

• Worked alongside the Centre’s staff members applying the principles of restorative justice to a wide variety of conflicts (including neighbor disputes, online harassment, theft, impaired driving, and elder abuse). Over the course of three months I was involved in all areas of the Centre’s restorative justice processes including receiving a file, pre-conference, resolution conference, as well as follow up after the creation of a resolution agreement. Elected to the board of directors following practicum placement.
